User Acceptance Survey
The User Acceptance Survey is a Likert-scale questionnaire administered to evaluate your system. Most BSIT/BSIS programs require it.

What this section does
Describes the survey instrument used to evaluate the system in Chapter IV. Most programs require this to be a four-point or five-point Likert scale tied to TAM, ISO 25010, or your chosen framework.
The four-point Likert scale (recommended)

Statistical treatment formula
Weighted Mean = sum(f x x) / n
f = frequency of each Likert response x = corresponding scale value (1, 2, 3, 4) n = total respondents
Mentor's tip
Pilot test with 5 respondents before the real survey. Items that everyone scores 4 are useless; items that confuse respondents need rewording.
Panel defense checklist
- [ ] Likert scale defined with verbal interpretations.
- [ ] Each item maps to one quality (ISO 25010 or TAM construct).
- [ ] Sample size justified (e.g. 30 = Slovin or convenience).
- [ ] Administration described (paper, Google Forms, in-person).
- [ ] Statistical formula stated.
- [ ] Survey instrument attached as an appendix (Appendix C).
- [ ] Ethical considerations noted (informed consent, anonymity).
Mentor’s tip: Use a 4-point Likert mapped to ISO 25010 or TAM. Define the verbal interpretation table. Pilot with 5 respondents. Attach the full instrument as an appendix.
